• Availbility of vehicles is limited, so there is no guarantee of receiving a vehicle.  
• Applications are good for one year.  
• Preference is given to applicants needing transportation to get to a job. 

1. Must be a citizen of USA.
2.  Live in Warren, Lincoln, Franklin, Montgomery, or St. Charles County
3. Demonstrate significant need.
4.  Submit an online or paper application. 
5.  Have a valid driver's license.
6.  Submit a sponsor's referral letter from a staff person at your church or another organization  that you are working with for support. 
7.  Submit proof of income:  2 most recent paystubs, if any.
8. Submit proof of financial assistance, if any.
9. Have ability to pay for titling of vehicle and liability insurance.  Wheels of Faith may be able to assist. 
10. Attend a finance class and a car maintenance class before or at time of purchase. 

1. I understand that this is an application for assistance to purchase a vehicle. Wheels of Faith does not sell vehicles.
2. I have attached my referral letter and supporting documents to this completed application. (See Requirements 2-7 above.)
3. I have answered all questions truthfully.  
4. I understand that there is no guarantee that I will be selected to purchase a vehicle through Wheels of Faith.
5. I understand that there is limited availability of vehicles. 
6. I understand that there is no warranty on the vehicle. The vehicle is purchased “as is” and will have  passed inspection.
7. I understand  that Wheels of Faith may require me to take classes prior to my taking ownership of a vehicle.
8. I understand that this application will stay active for 1 year.  At which time, if I still need a vehicle, I  must submit a new application.

1. Include the name and address of the sponsoring organization, and name and contact  information of person providing the referral. 
2. Describe the sponsor's relationship with the applicant,  AND  why they believe the applicant is a good candidate for our program. 
3. Review the application with the applicant, especially the Requirements and Understandings listed above, and on the first and last pages of the application.
